Emergency Care

Know what can wait — and what needs prompt attention.

Most unexpected orthodontic problems are uncomfortable rather than dangerous. A loose bracket, poking wire, damaged aligner or sore spot often needs timely attention but not an emergency room.

Call Bright Smiles when something breaks, stops fitting or causes unusual discomfort. If there is major facial trauma, heavy bleeding, difficulty breathing or swallowing, or another serious medical emergency, seek emergency care immediately.

Loose bracket?

Cover irritation with orthodontic wax if needed and call us. A loose bracket can affect treatment even when it is not painful.

Poking wire?

Try orthodontic wax and, if comfortable, gently press the wire toward the tooth with a clean cotton swab or pencil eraser. Call if it continues to irritate.

Broken or lost aligner?

Do not keep wearing an aligner that is sharp, badly distorted or painful. Contact us before deciding which tray to wear next.

Severe swelling or trauma?

Major facial injury, heavy bleeding, difficulty breathing or swallowing, or severe pain with swelling and fever needs urgent medical or dental evaluation.

MOST ORTHODONTIC PROBLEMS ARE URGENT — NOT LIFE-THREATENING

First decide what kind of problem you are dealing with

A broken bracket or irritating wire should not be ignored, but it is very different from uncontrolled bleeding, facial trauma or difficulty breathing. The first step is to identify whether the problem is an orthodontic appliance issue, an urgent dental injury or a true medical emergency.

If you are not sure, call us and describe what happened. We can help determine whether a temporary home measure is reasonable or whether you should be seen sooner.

COMMON ORTHODONTIC PROBLEMS

Problems that usually start with a call to Bright Smiles

These issues can interfere with treatment or irritate the mouth, but they are usually not medical emergencies. Contact us so we can give instructions based on the exact appliance and what happened.

Loose or broken bracket

If the bracket is still attached to the wire, leave it in place. Wax can reduce rubbing. Avoid hard or sticky foods and contact us so we can decide when it should be repaired.

Poking or loose wire

A long or sharp wire can irritate the cheek or lip. Wax can create a smooth barrier. If you can do so comfortably, gently press the wire toward the tooth. Do not cut it unless we specifically instruct you.

Loose band or appliance

Do not keep wiggling or pulling on a loose band, expander or other appliance. If it comes out completely, save it and bring it with you.

Lost, cracked or warped aligner

Do not wear an aligner that is sharply cracked, badly distorted or causing significant pain. Call before advancing to a different tray or returning to an older one.

Loose attachment or button

A missing aligner attachment or elastic button may change how force is delivered. Keep wearing the appliance only as instructed and let us know what came off.

Sore spots & irritation

Tenderness after an adjustment and small areas of irritation can often be managed temporarily with wax, soft foods and warm salt-water rinses. Call if pain is severe or worsening.

If the problem involves breathing, swallowing, uncontrolled bleeding, major facial trauma or a serious dental injury, your immediate safety comes before the orthodontic appliance.

WHEN ORTHODONTICS IS NOT THE MAIN PROBLEM

Situations that need urgent medical or dental care

These situations go beyond a routine appliance problem. Stabilize the injury or medical condition first; the braces, aligners or other orthodontic appliance can be assessed after you are safe.

Difficulty breathing or swallowing

Call 911 or seek emergency medical care immediately. Do not wait for an orthodontic appointment.

Heavy or uncontrolled bleeding

Persistent or significant bleeding from the mouth or face requires urgent evaluation.

Suspected jaw fracture or major facial trauma

A significant blow to the face, abnormal jaw position or concern for a fracture should be evaluated urgently.

Knocked-out permanent tooth

This is time-sensitive. Seek emergency dental care immediately. Keep the tooth moist and avoid touching the root.

Severely displaced tooth

A permanent tooth pushed far out of position or deep into the gums needs urgent dental evaluation.

Severe pain with swelling or fever

Significant facial swelling, fever or rapidly worsening pain may indicate infection and needs prompt dental or medical evaluation.

TEMPORARY COMFORT MEASURES

A few simple supplies can make minor problems easier to manage

Temporary home measures are for comfort while you wait for professional guidance. They are not substitutes for evaluation when an appliance is broken, pain is severe or symptoms are worsening.

Orthodontic wax

Useful for covering a bracket or wire that is rubbing the cheek or lip while you wait for definitive repair.

Floss & interdental brush

Helpful when food is trapped around brackets or wires and contributing to pressure or irritation.

Warm salt-water rinse

Can soothe irritated tissues. Use comfortably warm—not hot—water and do not rely on rinses to treat severe swelling or infection.

Small mirror & retainer case

A mirror helps you see what changed. A case or clean container lets you save a loose component, aligner or retainer and bring it to the visit.

HELP US TRIAGE THE PROBLEM

Three things that make the call more useful

Tell us what happened, when it started, how much discomfort there is, and whether the appliance is still attached or wearable.

Look at what changed

Use a mirror and good light. Is the bracket loose? Is a wire long? Is an aligner cracked? Is there swelling or bleeding?

Take a clear photo if you can

A photo can help us understand the problem before the visit, especially for a loose bracket, wire, attachment, retainer or aligner.

Save any loose part

Keep a bracket, band, appliance, aligner or retainer in a clean container and bring it with you rather than throwing it away.

FREQUENTLY ASKED QUESTIONS

Orthodontic emergency questions, answered clearly

Is a broken bracket an emergency?

Usually not a medical emergency, but it should be reported because it can affect treatment or irritate the mouth. Wax can help temporarily if it is rubbing.

What should I do with a poking wire?

Use orthodontic wax and, if it is comfortable to do so, gently press the wire toward the tooth with a clean cotton swab or pencil eraser. Call us if it remains sharp or painful.

Can I cut a poking wire myself?

Do not cut a wire unless Bright Smiles specifically instructs you to do so after understanding the situation. Small wire pieces can be difficult to control or swallow.

What if my aligner cracks?

If the crack is minor and the aligner remains smooth and fits normally, call for instructions. If it is sharp, distorted or painful, stop forcing it and contact us before changing trays.

What if I lose my current aligner?

Call us before deciding whether to move forward or return to a previous tray. The right temporary step depends on where you are in the sequence and how the teeth are tracking.

What can I do for normal soreness after an adjustment?

Soft foods, orthodontic wax for rubbing areas and a warm salt-water rinse can help. Follow package or medical guidance for over-the-counter pain medicine. Call if pain is severe, worsening or unusual.

When should swelling concern me?

Significant or rapidly increasing facial swelling, especially with fever, severe pain or difficulty swallowing or breathing, needs prompt dental or medical evaluation.

What if a tooth is knocked out during sports?

A knocked-out permanent tooth is time-sensitive. Keep it moist, avoid touching the root and seek emergency dental care immediately.

What if my fixed retainer breaks?

Call us. A loose bonded wire may allow tooth movement or irritate the tongue. Do not pull the wire off yourself.

Should I save a loose bracket or appliance?

Yes. If a piece comes out completely, place it in a clean container and bring it with you.

Can I still play sports with braces after a minor problem?

If the appliance is damaged, get guidance before returning to contact sports. A properly fitted mouthguard helps reduce injury risk during orthodontic treatment.

Should I go to the ER for a broken bracket?

A broken bracket alone usually does not require an emergency room. ER care is more appropriate for major trauma, uncontrolled bleeding, difficulty breathing or swallowing, suspected jaw fracture or other serious medical emergencies.
